Hampton, Va. - Police have confirmed that a woman has died as a result of a two vehicle crash on Greenbriar Avenue and W. Pembroke Avenue.
The victim has been identified as 55-year-old Teresa P. Jones of Hampton.
The call came in at 8:21 a.m. on Thursday, November 7, 2013.
Police say a Chevrolet sedan attempted to make a left turn from Greenbriar Avenue to travel West onto Pembroke Avenue, when it collided with a Chevrolet work van that was traveling East on Pembroke Avenue.
The driver of the Chevrolet sedan was pronounced deceased on scene. Her front seat passenger was transported by ambulance to a local hospital with non life-threatening injuries, police say. There were no additional injuries.
The crash is being investigated by members of the Hampton Police Division Fatal Crash Team.
The cause of the crash is still under investigation.
